1)  Form must be typed. Please use the ‘Print for submission to Court’ button at the top of the page to print instead of File, Print. If this button is not used to print, the form will be rejected at the Clerk’s Office.

3)  Filing Fee is $10.00.  (Checks can be made payable to County Clerk.)

4)  Can be filed in person at the Circuit Court Deed Room or mailed to:
